APPLYING FOR ASSISTANCE To be eligible, you must have HIV/AIDS and qualify for assistance based on your income. The income guidelines are as follows: monthly income for individuals must be $1,500 or less; for families, the maximum monthly income allowable is $1,500 plus the addition of $250 per family member. Please request your case manager contact us to receive the application and a listing of necessary documentation. Your case manager may then complete the application and supply the supporting documentation (proof of income, health status, etc.) and submit your case for consideration.
